Mobile Care Coordinator® is a robust combination of patient advocacy and high-touch care management. MCC is PROACTIVE, mobilizing nurses to respond immediately, in-person, when a covered individual is admitted to a hospital or other healthcare facility. Whether the health problem is acute or chronic, MCC nurses keep patients engaged in following the care plan, greatly improving outcomes and reducing hospital readmissions. Our hybrid program includes all Patient Advocacy services plus a shared, fractional MCC program. Eligibility is based on the number of covered lives.
Program pricing
No matter the size and needs of your patient population, Guardian Nurses can customize a Mobile Care Coordinator® Program perfectly suited to your organization. You pay one all-inclusive annual fee based on the number of nurses needed.
Typically, one MCC nurse is needed to serve 2,500 to 5,000 covered lives. For larger populations, two, three or four nurses may be needed.
Our hybrid program is designed for patient populations under 2,500. A hybrid program includes patient advocacy and shares an MCC nurse with one or more other hybrid programs.

How we work with unions and employers
Unions and employers seeking to save money while improving healthcare outcomes contract with Guardian Nurses as they do with any other healthcare provider.
It starts when we are invited to make a presentation to the employer’s HR team leaders or the board of the union health & welfare fund. We explain our services and cost structure, and the anticipated results in health outcomes and savings. We answer their questions, and they request a proposal.
Typically, Patient Advocacy can be up and running within a month and MCC within 3-6 months. Guardian Nurses works closely with the employer HR team or health & welfare fund administrative team and provides detailed quarterly progress reports.
