One all-inclusive annual program fee covers everything

No matter the size of your patient population, Guardian Nurses can customize a Mobile Care Coordinator® Program to meet your needs.

Typically, one MCC nurse is needed to serve an employer health plan or union health & welfare fund with 2,500 to 5,000 covered lives. For larger populations, two or three nurses may be needed. 

For patient populations under 2,500 we offer a hybrid of our MCC and patient advocacy programs. A hybrid program shares an MCC nurse with one or more other hybrid programs.
